Add documentation for 4 CLI flags that were implemented but missing from USAGE.md:
--effect-types— configure which API types to generate--server— set base URL for the OpenAPI server--no-elm-format— skip running elm-format on outputs--keep-going— skip ungenerable routes instead of erroring
