Add execution error reporting

• Adds a Workflow execution-error occurrence reporter for agent run observability.
• Routes reports through world.executionErrors instead of a generic Workflow observability namespace, keeping the surface scoped to runtime failures.
• Includes a changeset so the reporter can be published before Eve depends on it.
