docs(sbom): add policy decision examples

• add examples/policy-decisions with pass, warn, fail, and needs-review consumer examples
• document that needs-review is a consumer interpretation, not a runtime summary.policy.status value
• link the examples from policy decision docs and reviewer path
• add pytest coverage for example completeness and shape
• extend LF policy to JSON and Python examples/tests
