Documents two recent fixes in the staging provision runbook: 1. Bootstrap stall root cause + fix (reflectt-cloud#2345) — TEAM_INTENT env var not passed to node Fly machine on invite-path provision; both #2342 and #2345 are needed together. 2. Fly Machines API PATCH→POST fix (reflectt-cloud#2348) — bulk image update was 404ing; fleet update command now documented. Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>
