We previously had a hodgepodge of "pos", "loc", "span", "range", "section", and "group", and it wasn't always clear what was what. From now on, a "pos" is a single index into the file, and a "span" is a section of the file that starts at a "pos" and ends at a later "pos". Also unify type annotations to use the Span type.
