Index meeting summaries in MCP search

Summary

• Add a v3 MCP index schema with meeting summary search documents backed by FTS5.
• Index summary title, attendees, decisions, action items, and open questions from the shared CaptureSummaryParser.
• Rank summary matches ahead of raw utterance-only matches while preserving capped raw snippets when a meeting matches both.
• Force v2/v1 index rebuild/backfill on next reconcile.

• Product runtime remains local-only; no cloud calls were added.
