refactor: standardize server logging

What Changed

• Migrated Codex manager lifecycle logs from console to Effect.log.

Why

• Log Persistence: Events logged via console are not captured by the project's
  rotating file sink. This change ensures critical provider events are persisted to disk.
• Consistency: Brings the Codex server management logic in line with the established
  patterns used by the Git and Terminal services.
