Map removed root commands to mounted replacements

This migration block says flat root commands were removed, but two rows map commands to themselves (specfact code repro ... and specfact govern enforce ...). In upgrade scenarios, readers coming from pre-0.40 root forms (specfact repro ..., specfact enforce ...) do not get the actual replacement paths, so they can still copy commands that no longer exist on the current mounted surface.
