planning/decisions/ADR convention — one file per design decision taken (especially options rejected with a load-bearing reason), each with a revisit trigger, surfaced byjust index. Ports the convention already shipped in modern-di and faststream-outbox.planning/_templates/decision.md— the ADR templateplanning/index.py— generated index now renders a## Decisionssectionplanning/README.md+CLAUDE.md— convention proseplanning/decisions/.gitkeep— seeds the directoryMechanical convention sync; no code change.
