This PR resolves #11 and #12. It makes sure the code changes apply cleanly to the new base.
In addition, because it updates some of the HTML attributes on the form elements, it also makes sure we do not have duplicate HTML
idvalues. This was never compliant with the HTML spec.Main cred to @btrem!
