refactor(llm): remove dead provider helpers

Summary

• remove the unused BaseLLM stop-word implementation helper
• remove unreachable synchronous and asynchronous Anthropic follow-up conversation helpers
• remove unused Bedrock Nova and media-format helpers
• remove the test that directly exercised the otherwise unreachable Anthropic private method

Why

Repository-wide reference tracing found no production call sites for these private helpers. The Anthropic synchronous helper was only referenced by its unit test and was already marked with a TODO to remove it. Active tool execution, stop-word handling, multimodal detection, and file-upload paths remain unchanged.

This is PR 1 of a planned seven-PR dead-code cleanup, split by subsystem to keep review and regression scope focused.

Impact

No public API or active runtime behavior changes. The cleanup removes 287 lines across the LLM base class and Anthropic/Bedrock providers.

Validation

• mypy passed for all three changed production modules
