Revamp README layout and add logo asset

Rewrite README to a redesigned, concise documentation layout: adds centered logo and badges, reorganizes Quick Start, Feature Highlights, Targets, Installation, Requirements, Examples, Development, Docs and License sections, and clarifies v2 hook/migration notes and transformer/lossy behaviors. Removes large legacy prose and example blocks in favor of compact examples, tables, and links to MIGRATION.md/ROADMAP.md. Also adds assets/codablekit-icon.svg as the package logo. This refresh improves discoverability, streamlines onboarding, and surfaces important v2 changes and test/build instructions.
