Adds the cdm55changes page to the documentation. Added folder with sql scripts for multiple dbms to migrate existing cdm in 5.4 to 5.5.
Updated the cdm54changes page to have the same structure.
For consistency, also created the page of cdm53Changes and v5.2->v5.3 migration scripts.
The existing v5.3->v5.4 scripts were moved to subdirectory.
The v6.0->v5.4 scripts were moved to extras/archive folder.
