Add a preface that this IG defines how ACP health information is accessed and structured, and that addressing, routing, localization, consent management, and authentication are provided by underlying infrastructure/specs. Rework the General API requirements: move and rephrase Authorization and Resolving References, and expand Patient Context with examples for obtaining the patient's logical ID (including an IHE PDQm ITI-119 reference). Minor wording and formatting adjustments for clarity.

Should not conflict with #103
