Add patient's screen

✨ Feature: Patient Detail Screen

Summary

This PR adds a new PatientsScreen to the app, allowing therapists to view and manage details for individual patients. The screen is navigated to from the PatientCard on the HomeScreen.

📱 Features

• Displays patient's name and ID with an avatar.
• Presents three actionable cards:
  • Tailored Goals
  • Daily Activities
  • Development Milestones

🛠 Implementation Details

• PatientsScreen is a stateless widget with two required parameters: patientName and patientId.
• Uses SafeArea, Padding, and ListView for structured layout.
• Custom _buildCard() method provides reusable card UI for patient categories.

✅ How to Test

1. Run the app.
2. On the HomeScreen, tap on “View or Manage Patient” on any PatientCard.
3. Verify the navigation and correct display of patient details and category cards.
