Broken icon displayed on migrator page when a database error occurs

Describe the bug
When a database error occurs on the migrator page, the icon shown in the UI is broken or not rendered correctly.

To Reproduce
Steps to reproduce the behavior:

1. Open the migrations/migrator page in a Wheels application
2. Trigger a database error
3. Observe the UI after the error is displayed
4. Note that the icon shown alongside the error is broken or missing

Expected behavior
When a migration error occurs:

• A proper error icon should be displayed
• The icon should render correctly and consistently
