feat: generate 295 repo summaries + fix ingest script + add outcomes/recipes

- index.json: auto-generated summaries for 295 repos (was null)
- ingest-scan.sh: fix root cause — generate summary from scan data
  instead of reading nonexistent $scan_data.summary field
- outcomes: 2026-03.jsonl (9523 records) + fleet-import (6016 records)
- dispatch: manifests for 2026-03-06/07 + held queue
- recipes: 20 scorecard recipes + race-condition, resource-leak, unchecked-alloc
