Please include:
- VS Code version
- Operating system and version, and whether the agent runs through WSL
- Extension version
- The coding agent you launched and its configured command
- Whether that command works in a regular VS Code integrated terminal
- Whether the agent appears under Ready, Agents, or Setup required after refreshing
- The relevant status from Super CLI: Run Agent Doctor (the report already excludes environment values)
- Relevant terminal output, without secrets or authentication tokens
