The smithers-orchestrator npm package was renamed to smthrs (0.33.0, published 2026-08-02, no compat alias under the old name), which affects the pinned fleet tool in scripts/fleet/setup/external-tools.json and scripts/fleet/setup/lib/install-smithers.mjs.
