What happened?
Birch (triage agent, ID 1b40de43-12c5-4d5b-acea-68010e28bc97) in rig d7883019-a940-46d3-9729-a9a0add06810 repeatedly escalates with "Triage agent cannot resolve triage requests - role mismatch" or "permission error".
The patrol keeps spawning triage batch beads every ~90 seconds, Birch picks them up but cannot resolve them (role mismatch), which triggers an escalation, which spawns another triage request, creating an infinite loop. This results in piles of failed triage beads (we've had to delete 20+ in a single cleanup).
Previous escalations from this same issue:

- 2026-06-02T14:22: "Triage agent cannot resolve triage requests - role mismatch"
Root cause: Birch's agent role does not have the permissions needed to resolve triage request beads (gt:triage-request label). The triage agent should either be granted the correct permissions, or the patrol should stop spawning triage batches for agents that can't handle them.
